Eligibility Assessment

We’ll start with a friendly phone chat to understand your needs and see if you meet the current NDIS eligibility requirements.

Participant Meeting

A brief and friendly session to introduce our services, walk you through the process, and answer any questions, ensuring you feel informed, comfortable, and confident moving forward.

Service Match

Our service delivery team, led by a Nurse, will review your needs and goals to confirm that we can provide the right supports and services for you.

Service Agreement and Consent

The participant reviews the Service Agreement and provides their consent. Once both the participant and Silverlining SA agree to the terms, the services are officially ready to begin.

Service Launch

Our service delivery team will begin creating the individualized care plan, organizing any required training for the support team, and commencing services. All supports will be tailored to the participant’s needs and preferences & adapted as needs evolve.
